TAG's Aftercare Program consists of
- A 3-6 month transition aftercare program that provides a local support infrastructure for teens, ages 12-18, graduating from a therapeutic program and returning home
- A weekly group session that helps facilitate continued emotional growth and integration of the teen's therapeutic experience into his/her daily life
- A safe place for teens to process the issues related to their transition back home without shame or embarrassment from where they have been
- A place where this unique group of teens can continue to hold themselves accountable for their choices, while having their peers and a licensed professional counselor check and balance their actions through weekly feedback
- A safety net to guard against the teen's vulnerability and increased risk of falling through the cracks once back home
TAG's Aftercare Program offers the following
- A transition aftercare group for adolescents, facilitated by a Licensed Professional Counselor
- Multidisciplinary communications with key professionals (program liaison, therapist, educational consultant, school counselor)
- Weekly monitoring of teen's progress
- Appropriate recommendations/referrals for additional support
- Educational sessions that reinforce a healthy and positive way of life (i.e. decision making, triggers/relapse, redefining peer groups, interest development, anger management, home contracts, coping skills, future planning, academic challenges/goals, use of leisure time)
- Ongoing, professional support and guidance
